The defendant in the above-captioned matter hereby moves this honorable Court for an Order requiring the plaintiff and the attorney for the minor child (if one is appointed) to file with the Court and serve on all counsel within one (1) week of the date of the granting of this motion, a UCCJA affidavit, as required by Conn. Gen. Stat. 46(b)-99, with respect to the addresses of the minor child for the past five (5) years and the names of the persons with whom the minor child has resided for the past five (5) years, including the addresses of the plaintiff and the minor child, whether temporary or permanent.
